(Participation in the IPA MAP survey is REQUIRED for participation in the IO surveys.)

If you’d like to participate in the IO surveys, you may request the survey forms. (This survey and corresponding reports are optional and you will receive a complimentary executive summary of each report, on firm administration, human resources and information technology. If you would like to purchase the three IO Reports, you may do so at a preferred rate.)
